SCHOOL VACCINATION REQUIREMENTS PER STATE LAW
Lolo School Parents –
Recent changes in state laws are requiring schools to be stricter in the timeliness of receiving required student immunization forms. Lolo SD has provided a bit of extra leeway this year with an approximate 10 day grace period (September 9th) give the law change. However, after that date, students without completed immunization forms will, by law, not be permitted to attend school.
The vaccinations listed below are required in order for your student to attend school. Please make sure to provide the school with your student’s documented immunization information before the student starts school! A vaccination form is available from the school office, the school website, or your health care provider/Missoula County Health Department. By law, without this form, students may not attend school UNLESS they qualify for one of these three exceptions (each of which require the parent/guardian provide the school with a completed form which are also located on our school website or at the school office):
- Conditional Attendance: As long as the student has received at least one dose in the series of required shots, a Conditional Attendance may be allowed if the Conditional Attendance form is completed and presented to the school office.
- Medical Exemption: Must be completed by a physician and done only in cases where there is a medical contraindication. The exemption will specify what vaccine(s) is being exempted and for how long. The completed form must be submitted to the school office.
- Religious Exemption: A religious exemption may be granted if the appropriate form is completed/submitted to the school office. The parent must sign this form in the presence of a notary and bring the notarized and completed form to the school at the start of each school year. The form must indicate which vaccines are being exempted.

Immunization Requirements (our thanks to MCPS for their helpful website information)
Children should be immunized against the following communicable diseases. FOR SCHOOL ENTRY, the minimum requirements are:

1 - Per MCA 20-5-402 a preschool is defined as a facility that provides, on a regular basis and as its primary purpose, educational instruction designed for children 5 years of age or younger and that: (a) serves no child under 5 years of age for more than 3 hours a day; and (b) serves no child 5 years of age for more than 6 hours a day.
2 - Hib vaccine is not recommended for children older than 59 months.
3 - DT vaccine administered to pupils less than seven years of age is acceptable only if accompanied by a medical exemption that exempts the pupil from pertussis vaccination per ARM 37.114.705.
4 - A pupil 7 years or older who has not completed the DTaP requirement must receive additional doses of Td/Tdap vaccine to reach a minimum of 3 doses of any combination of DTaP, Td, Tdap or DTP per ARM 37.114.705.
5 - While it is not recommended, if a child younger than 13 years receives their second dose of varicella at an interval of 4 weeks or longer, the dose does not need to be repeated.
6 - As of October 1, 2015 pupils are required to have varicella vaccine and all pupils 7th-12th grade must have a Tdap vaccine.
